NTC Community Health Nurses (Macoah)

NTC Community Health Nurses can support you and your family with the prevention of diseases that are spreadable. These diseases are called Communicable Diseases and include things like tuberculosis and sexually transmitted diseases. As your nursing team, we can help you understand how to prevent these diseases and how or where to get treatment for the diseases.

The Community Health Nurse (CHN) provides support during pregnancy and birth to establish positive and healthy birth outcomes using the Mother’s Story approach to care. The CHN identifies the physical and emotional needs of parents and babies in the first weeks of life and beyond and fosters bonding and attachment of mothers and babies. The CHN provides assessments at various stages of life, education and referrals as needed.
